Standard Fee My standard fee for a regular 50-minute therapy session for an individual, couple, or family is $100.

I understand that it can be difficult to pay for therapy, and times are particularly hard. I always do the best I can to work with what each person can afford, which is something we can discuss in the initial, 30 minute, complimentary, consultation meeting.

Payment All session fees are due at time of service. Payment is by cash or check (payable to: Jennifer Musty Rives) and is due at the beginning of each session. All charges incurred are the responsibility of the client.

Insurance I choose to provide fee-for-service therapy. What happens in our work together is shared between us, instead of with an insurance company. In order to be reimbursed from insurance companies, you need to be diagnosed with a mental health disorder diagnosis from the DSM-IV-TR. As a fee-for-service provider, you do not have to receive a diagnosis. This also means that insurance companies will not have access to your treatment plan, case notes and other important and private mental health information. This way we can focus on the important thing, our therapeutic relationship.

Health Savings Accounts you may be able to make use of a Health Savings Account, or Flex Spending Account. Please speak with your provider to determine requirements. I’m happy to provide you with a receipt.

Cancellation Policy I require a 24 hour cancellation notice for any scheduled appointments, unless in case of emergency. If you have not notified me at least 24 hours in advance, you will be required to pay the full session fee before the next appointment. Clients who are late to scheduled appointments will be charged for the full session.

Note: Fees may change at any time without notice. I will make every effort to work with existing clients, who will be alerted in advance of any fee increases.

Forms.

Before your visit ...

Below are some important documents for you to take a look at. The first form is an intake questionnaire for you to fill out, which will help me better understand how I can be of help to you. The second form is a therapy agreement, that clearly outlines our professional relationship. The last three are documents regarding your rights as a client, your confidentiality, as well as privacy practices. If you have any questions regarding any of these forms, please don’t hesitate to contact me.

Please print out the following forms, complete, sign, and bring them with you to your first session.
